Welcome to the 2025 Together We Can Annual Report

Growing up has never been easy, but today’s teens and young adults are navigating a more complex world than ever before. From the isolating effects of digital media to a rapidly changing economic landscape, youth disconnection is a growing challenge—not just for at-risk teens, but across the general youth population.

At the Together We Can Foundation, we believe in being the “ground crew” for the next generation. Our mission is to equip them with the life-work success skills they need to build a launchpad for a thriving, independent adulthood.

We are thrilled to share our December 2025 Annual Report, which highlights the incredible milestones our community has achieved together. Inside, you will find:

• 15 Years of Impact: A look back at 15 years of our Smart Transitions program, which has provided success skills training to over 13,300 youth across Hampton Roads.

• Empowering Resources: An overview of our Smart Guides to Adult Life (over 82,000 distributed to date!), including a preview of our upcoming Get Healthy guide.

• Real Outcomes: Inspiring data showing significant improvements in high school graduation rates, employment, and youth resilience.
